PROCEDURE TO ADD, RELEASE AND TRANSFER PLAYERS
Managers need to collect and return all paperwork to the SRU Club Registrar

ADD PLAYER
Complete the following:
1. CYSA Membership form – Form 1601
2. US Club Membership form – R#002
3. CYSA Team Roster Change in Status – Form 1606, Section #1
Return these forms to the Registrar along with the following:
1. A Copy of Birth Certificate or other approved birth forms
2. SRU on-line Registration Confirmation
3. A small photo for the CYSA player pass
4. Payment
U11 – U13/ $166 (this includes registration of $50 and 1st month payment of $116)
U14 – U18
Dec $245 (this includes registration of $100 and 1st month payment of $145)
March $195 (this includes registration of $50 and 1st month payment of $145)

Players will then pay the monthly amount at the beginning of each month for the rest of the season.
If players want to pay with credit card online they will need to call the SRU office at 541-7627 and do the payment over the phone. They will receive a confirmation email of their payment which needs to be included with paperwork turned in for proof of payment before they can be registered.
IMPORTANT NOTE:
A new PLAYER ADD is required by CYSA regulation to serve a seven day waiting period before they are eligible to play. This date is determined by the date that the District Registrar signs off on the paperwork.
RELEASE PLAYER
1. Complete CYSA Team Roster Change in Status – Form 1606, Section #2
2. Return form along with

TRANSFER PLAYER
Complete the following:
1. CYSA Membership form – Form 1601 – this should be obtained from the team the player is currently registered with.
2. US Club Membership form– R#002
3. Complete CYSA Team Roster Change in Status – Form 1606, Section #3,
This form requires signatures from the parent, old team official, new team official, and if the player is transferring from a league outside of Santa Rosa, then the releasing League Official.
Return these forms to the Registrar along with the following:

Number of CYSA Transfers that are allowed
U9 – U13 3 transfers on to your team. No limit to the number of transfers off
U14-U18 3 transfers on to your team. No limit to the number of transfers off.
In addition, 2 more transfers may be added for STATE CUP ONLY for a total of 5 transfers. Transfers 4 & 5 require the player/parent/team to complete a separate form that acknowledges that the player is only eligible to play in STATE CUP.
